The Church of Saint Anne in Horní Slavkov is a Roman Catholic hospital church. The third building with the same dedication now stands on this site. The oldest was a chapel of Saint Anne on the northwestern edge of the city on the path to Loket, on a slope above the Stoka stream valley.
